One of the harder truths about parenting is there might not be enough caution, baby-proofing or monitoring to fully ensure your child’s safety. There is always the risk that the split second your vigilance is lowered even slightly is the time when some accident befalls your precious little bundle. There is also the terror which grips parents at the thought of having their children far from them. But kids have to go to school and parents have to go to work. Parents constantly question whether their children’s minders and teachers have the skills to deal with a medical emergency in the event that it arises.

Dr. Irena Mehandjiska

Truth be told, anyone short of a licensed medical professional might not have all it takes to fully deal with arising emergencies. But even the average loving parent or caring teacher can learn how to mitigate a potentially deadly situation until a licensed health professional can attend to the child. The International Medical Center Beijing, located at the Lufthansa Center near LiangMaQiao subway station, is one place they can learn such skills. This past Sunday, January 12, the center hosted its first of a series of first aid training classes for parents, parents-to-be, and educators alike. Participants were taken through the paces of dealing with an emergency, from administering CPR to the dressing of wounds and stabilizing of dislocated and broken limbs.

The next such training will be held on February 22 at the IMC Center. You can book your place by either emailing the address provided or scanning the QR code to secure your place.

But that is more than a month away. In the meantime, and leading up to the Chinese New Year festivities when merriment might come at a cost to the little one’s safety, it is important to have some information on hand. Dr. Irena Mehandjiska from IMC lays out simple information on what to look out for, and what to do in the event that the unthinkable happens. Dr. Mehandjiska is a General Practitioner, Family Medicine where she deals with emergencies and continues to provide first aid training for parents, teachers, and caregivers.

What are some of the most common emergencies to be expected by teachers and parents of small children?

The most common emergencies in children are divided in two groups. In the first group are the medical emergencies such as fever, meningitis, choking, allergic reactions, poisoning, and injuries. In the second group are the environmental emergencies such as sunburn, heat exhaustion, heat stroke, bites, stings, and drowning.

What standard first aid techniques should parents have on hand for this CYN period?

Standard first aid techniques and the most commonly used are CPR and the Heimlich Maneuver. CPR stands for cardiopulmonary resuscitation. CPR is a lifesaving procedure when a baby/child’s breathing and heart has stopped. It is performed after drowning, choking, or other injuries. The Heimlich Maneuver is a first aid skill that is performed to someone who is choking and has stopped breathing, in order to dislodge whatever is causing the choking.

How does one administer first aid and/or CPR to a child whose airway is constricted?

CPR begins with placing the child on a flat, firm surface and asking someone to call an ambulance; and you need to check the scene for safety. Then you need to check if the child is responsive, and you need to see if the child is breathing or has any body movement. If the child is unresponsive and not breathing you need to begin CPR. Begin CPR by placing the heel of the hand in the center of the chest. Cover the hand with the other hand and start giving compression by leaning over the child’s body and locking the arms. Give 30 compressions with a speed of 100 per minute. After this step, open the airway by lifting the chin and tilting the forehead, pinch the nose, and place your mouth over the mouth of the child and give two rescue breaths. If the child is still not breathing you need to continue with CPR until another rescuer takes over or the medical help arrives.

Tomorrow (January 6), Dr Pik To Cheung, a pediatric specialist from Hong Kong with over 30 years of experience, will be visiting International Medical Center-Beijing to offer free pediatric endocrinology consultations for children aged 6-15.

But what exactly is pediatric endocrinology? What does a consultation look like? And which child populations are susceptible to the diseases associated with the endocrine system?

We spoke with Dr Cheung to find out, and to ensure that your child gets the help they need.

Can you give us a brief explanation of pediatric endocrinology (PE)?

Endocrinology covers all the physiological systems and functions served by hormones made in various glands (gonads, pituitary gland, thyroid gland, parathyroid gland, adrenal gland, specialized islets in pancreas).

Major endocrine functions known to the general public will be sexual function (in PE, that could translate into the determination of sexual differentiation in developing fetus, precocious puberty, delayed puberty and transgender or gender dysphoria related subjects), hypothyroidism or hyperthyroidism, diabetes, hypercholesterolaemia, hypoglycaemia, menstrual dysregulation in adolescent girls, virilization in girls, gynaecomastia in boys, osteoporosis, rickets, etc.

What does an endocrinology consultation entail? What kinds of tests should a parent and their child be prepared to undergo?

Detailed clinical history relevant to the presenting symptoms (see below) will be obtained from the patient, parents and caretaker(s). Appropriate physical examination will be conducted for the list of possible endocrine disorders responsible for the presenting problem. Measurement of height, weight, blood pressure and other physical parameters will be performed. Assessing whether a child has developed pubertal features (like breast or penile enlargement, body hair and acne, pubic hair etc) is performed when appropriate.

Tests could be conducted on blood, urine or special bodily fluid/specimen for general biochemical indices, specific hormonal levels, and lipid, protein or carbohydrate metabolites depending on the likelihood of suspected clinical disease derived from the review of clinical history and physical examination findings.

As each normal endocrinological subsystem hinges on maintaining the balanced and desirable physiological functions it serves, the confirmation of an abnormal endocrinological state may entail performing special dynamic tests which usually involve repeating assessments of key indices before and after a designed test agent is administered.

In addition, imaging of body organs/structure, for example by X-ray, ultrasonography, MRI or CT scan may be warranted to confirm or rule out the aforementioned differential diagnoses.

Who should seek out an endocrinology consultation? Is this something every parent should be doing for their child, or are certain groups more at risk for endocrine complications?

Examples of problems leading to endocrine consultation include:

short stature (long standing, recent poor height gain),

early puberty (breast enlargement, penile growth, pubic hair),

weight loss (especially associated with no loss of appetite)

weight gain

increase in urination (amount and frequency), but could be enuresis

midline neck mass (especially with mood change, palpitation, tremor and/or weight loss)

unusual/recurrent bone fractures

yellowish skin lumps

Well known at-risk groups for endocrine disorders as complication are:

Obesity

Infant born small for gestational age (SGA)

Those who have (and survive the harsh treatments for) childhood cancers, serious chronic inflammatory disorders requiring long term treatments

In general, it is good practice to keep a record of the developmental milestones of our children and consult the family doctor if some of these parameters are suspected to fall out of range.

Proactive discussion with doctors for possible related problems in families with strong positive history of metabolic syndrome (type 2 diabetes mellitus, hypertension, hypercholesterolaemia, obesity), dyslipidemia, endocrine related tumor, etc. is strongly encouraged in a society with advanced medical systems.

What will a parent learn from the consultation?

Parents should be able to define the scope of problems they have observed. The specific diagnosis could be clearly identified by the end of consultation, therefore the line of appropriate treatment could be outlined and a prospective plan explored and agreed upon under the guidance of the doctor. Often, the patient and parents will be given chance to get a pragmatic background understanding of such problems.

On the other hand, when the underlying cause of the defined problem is not yet firmly established, a line of workup which helps clarifying the diagnosis and relevant management plan will be suggested.

To book a consultation, scan the QR code, or call the telephone number provided

What are the risks of leaving an endocrinological disease untreated?

Certain endocrine disorders, when missed, could lead to life-threatening complications. These include type 1 diabetes mellitus, adrenal insufficiency, profound hypoglycaemia due to hyperinsulinemia, or inborn error of metabolism.

Others, if the optimal management time windows are missed, may lead to short final adult height, compromised bone health like osteoporosis with fractures, poor cardiovascular health with future heart attacks/stroke, and increased morbidity related to the primary endocrine problem.

What are the root causes of endocrinological diseases? What preventative measures can be taken to mitigate the onset of these diseases?

For a whole range of specific PE disorders, the root cause could be mutation of one single gene.

Examples could be genes involved in regulating the development of sex glands (gonads), thyroid or parathyroid gland, pituitary gland, endocrine pancreatic functions or bone metabolism.

In a subgroup of other PE disorders, it could be autoimmune in nature (for example, type 1 diabetes mellitus, Grave disease, Addison’s disease etc)

Yet some others could be nutritional, tumor, trauma, infection or inflammatory in origin.

Not many endocrine disorders are preventable. Notable examples will be obesity related complications like type 2 diabetes mellitus.

Are you seeing a rise in pediatric endocrine complications? If so, why?

There is a worldwide trend in rising incidence of type 1 diabetes mellitus. It must be related to environmental factors yet to be clearly identified.

Rising incidence of type 2 DM witnessed by us in Hong Kong and mainland China corresponds to increasing rates of childhood and adolescent obesity.

We have to take care of more endocrine complications in kids who have survived their cancer and serious haematological or chronic inflammatory diseases (mainly because of the successful treatment for their primary medical problems)

You’re based in Hong Kong, why is it important to bring these services to Mainland China?

I have witnessed strong, steady development in paediatric endocrinology all over mainland China in these past two decades. Despite that, I realize there continues to be significant demand from families coming over to Hong Kong for such service. This is not convenient for most families because of significant travel involved. I aim at providing an alternative option of bringing quality evidence-based practice closer to families who treasure our premier personal care in this field by establishing a workable system at BJIMC. Once this runs smooth, additional paediatric sub-specialties with similar conviction will be made available through BJIMC.

IMC is located at S-106/S-110 Beijing Lufthsansa Center, No.50 Liangmaqiao Rd, Chaoyang District.

However, celebrating multiculturalism here in China is easier said than done.

You may have seen or heard of local online articles that talk about racism or prejudice in China, both on the sides of foreigners and locals. There are polarizing opinions on that issue, but it becomes contentious once it reaches the vulnerable minds of children whose identities and perceptions of the world are still being formed.

For this edition of Doctor’s Corner, we invited Dr. Mike Mehrvarz, a child and adult psychologist at the International Medical Center (IMC), to talk about multiculturalism and its links to the mental health of children.

How do you define a multicultural child?
Dr. Mehrvarz: A multicultural child could be either a child born with parents from different nationalities with cultural uniqueness, or a child spending the most part of his or her life growing up overseas and exposed to local culture.

What are the issues associated with multiculturalism and what effect do they have on the emotional and mental health of children?
Dr. Mehrvarz: Aside from the parenting style, which is important for children’s normal psychological development, multiculturalism can become quite challenging for children. Of course, this highly depends on the mode of parenting, especially when parents make their children feel comfortable in a new culture and themselves comfortable as well. It’s hard for a child to accept a diverse culture if his/her parents themselves don’t feel comfortable and often complain or bring up negative issues about the local culture they are living in.

Childhood is quite a challenging journey, and if you add multicultural factors such as the child not feeling they are accepted, this could lead him/her to develop anxiety and depression. Sometimes, such emotional manifestations can lead to a learning difficulty, anger, fear, behavior problems, and more.

What kinds of issues do mixed-race or multicultural children face?
Dr. Mehrvarz: Human beings sometimes have a tendency to become part of a group of similar people and may, deep inside, reject other groups based on race, habits, and cultural issues, among many other factors. This might be due to the universal sense of insecurity humans may feel. And in order to feel strong and safe, we need to join groups whose members have similar experiences to ours. Of course, in our current civilized world, some try to change that tendency, but one cannot deny that its results exist almost in every culture. The mode of rejection, in this case, is due to racial issues which can lead to what we call “prejudice”. Now, children who are exposed to such resistance or prejudice from other groups obviously may develop a wide range of emotional difficulties such as low self-confidence, depression, anxiety, and possibly even learning difficulties. However, if the parents of these children are emotionally secure, psychologically grounded, have a healthy sense of self, and teach their children such attributes, then these kids would be far less impacted emotionally.

What kind of issues do Chinese children experience when they start living abroad?
Dr. Mehrvarz: From my experience living in China for over two decades, some children are not psychologically prepared and or not old enough to deal with culture shock, isolation, and the struggle to fit in, which every child can face when suddenly living in a new culture. Children need to live with their parents till the age of 18 to develop a healthy sense of self, identity, and happiness. Otherwise, these children may develop a wide range of emotional problems, which includes low self-confidence.

What kind of opportunities can we derive from multiculturalism?
Dr. Mehrvarz: The positive issues which can derive from multiculturalism can include the child becoming part of the global human race, meaning they feel comfortable with various cultures and habits, and enjoy the positive factors which derive from all these colorful cultures. In adulthood, these children can adventure to any corner of the globe feeling comfortable, excited, and able to face the many challenges they encounter.
